         Print  and include this page with the tape when sending!

3. Signed Diving Score Sheet

4. Diving Meet Results-listing of all the divers and their places (1 Page Only)

5. Video Tape / DVD

6. FEES FOR NON NISCA MEMBERS
           $30 for first application
           $50 for two applications

6. FEES FOR NISCA MEMBERS
           $15 for first application
           $25 for two applications

7. Late Fee $20 per application
          (Applies if submitted after season deadline.)
          Fall season deadline – December 31st
          Winter   season deadline – March 31st
          Spring season deadline - May 20th.
          If your state championship is after this deadline, contact the All-America Diving Chair.

8. Make checks payable to NISCA or please include the receipt if paying by PayPal.

9. The Diving Portal will close at midnight May 25th.
         Applications will officially be closed at this time.
